Hours: 40 hours per week, Monday - Friday, 8:00 a.m. - 4:30 p.m. |

Job Description
Nature of Work: Engage, educate and support pregnant and postpartum women and women with children to make life changes necessary to recover from disabling mental illness and/or substance use disorder conditions. Facilitate the care of and streamlining handoffs with other parties in the county health care system, ensuring that participants receive the appropriate continuing care. Work with families on ensuring they receive public benefits and other supportive services for the families. Complete and oversee all data requirements for the Strengthening Families in Recovery Program, as well as the completion of the Government Performance and Results Act (GPRA) on all enrolled individuals. Key tasks: Promote timely access to appropriate care. Assist clients with navigating the service delivery system. Create and promote adherence to a recovery plan, developed in coordination with the client and when appropriate, providers and family/caregivers. Facilitate client engagement and follow-up in treatment. Increase the client’s ability for self-management and shared decision-making. Connect clients to relevant community resources, with the goal of enhancing client health and recovery. Assist in the expungement of criminal records. Support families that need documents/certificates from vital records (i.e., birth certificates). Transportation for job appointments, school appointments, medical, mental health and other appointments that benefit the family. Provide targeted individual and group level education and outreach for all services along the substance use continuum of care. |
Minimum Qualifications
Education: Associate degree in behavioral health from an accredited college or university. |
Experience: Must be able to provide a personal statement describing what skills and abilities he/she has to offer to this field of work; maintain daily contact notes and submit monthly reports timely and accurately; and coach and model effective coping skills and develop a relationship of rapport and trust. Strong communication skills with other health professionals, coworkers and clients. Excellent computer skills to include Microsoft Word and Excel. Professional demeanor. |
Notes: Required to possess a motor vehicle operator's license valid in Maryland and have access to a personal vehicle. |
Desired or Preferred Qualifications: Familiarity with treatment and community resources that support recovery from addictive diseases and co-occurring behavioral health disorders. Ability to maintain effective peer recovery relationships with participants and other members of the team. Exceptional organizational and time management skills. Experience with technology, specifically Gmail, Google Calendar and Google Drive. Excellent and situation-appropriate oral and written communication skills. |
