Location: 6701 Baymeadow Drive, Suite B, Glen Burnie, MD 21060
Hours: 40 hours per week
Benefits: Participation in the County’s group health plan including prescription, dental, and optical, paid holidays and sick leave. Optional deferred compensation program at employee's own expense.

Job Description

Nature of Work:

Receive incoming client referrals, process them, and reach out to eligible clients to enroll in the Healthy Start Program. Provide short-term care coordination as assigned, identifying and connecting clients to community resources as indicated. Provide translation services to Spanish-speaking clients while accompanying Healthy Start Program nurses, parent educators, and/or mental health staff on home visits or on-site as assigned. Conduct outreach activities in the community to private individuals, health care providers, and community organizations to educate them about the Healthy Start Program and obtain referrals.

Minimum Qualifications

Education: 

High School Diploma or Equivalency (GED)

Experience:

Six months of related experience in care coordination services OR Possession of an associate's or bachelor's degree from an accredited college or university in education, social work, psychology or a related field.

Notes:

Spanish-speaking is required.

Desired or Preferred Qualifications:

Experience in care coordination services.
