Reports are due by the 10th of each month.

  • Narrative Report*
  • Performance Measures*
  • Signed Timesheets* 
  • Signed mileage logs (if applicable) 
  • Signed invoices by the authorized signatory for this program (invoices include staffing, mileage, other items, and special projects)* 
  • Receipts for purchases (if applicable) 
  • Payroll Registers or pay stubs for W-2 employees and copies of checks for 1099 employees
  • FY25 Relationship Management Form 

The asterisk (*) indicates that these components are required for the submission of your monthly report.  If any of these components are missing, your report is considered incomplete.
