Salary: $17.00 – $18.50 per hour
Benefits: Participation in the county’s group health plan (including dental, prescription and optical), paid holidays, annual leave and sick leave. The selected employees may also participate in the county’s deferred compensation program at his or her own expense.
Nature of Work: The Care Coordinator will provide non-clinical assistance and recovery support, while maintaining follow-up with individuals who have utilized crisis services. The Care Coordinator will assist individuals with navigating the service delivery system, accessing wrap-around services using a multisystem approach. The goals to this approach are to improve outcomes for individuals who have utilized crisis services and who are opioid dependent and/or have other substance use disorders.
Associate’s degree in the human services field. Two years of experience providing high volume peer support coordination of care services in lieu of associate’s degree will be considered.
Experience: Two years case management experience working with the guiding principles of recovery that assist individuals to improve their health and wellness, live a self-directed life, and reach their full potential. Reliable transportation required.
One position works with individuals in substance use disorder crisis. Hours are either Saturday through Wednesday or Sunday through Thursday.
One position works with women with children/pregnant women that have opioid disorders. Hours: Monday through Friday 8:00 a.m. to 4:30 p.m.
