Salary: $29.40 per hour
Benefits: Participation in the county’s group health plan (including dental, optical, and prescription), paid holidays, annual leave and sick leave. The selected employee may also participate in the county’s deferred compensation program at his or her own expense.
Nature of Work: To perform in-home assessments for a variety of programs that serve people with disabilities or seniors in the community. Perform interRAI assessments and Statewide Evaluation and Planning Service (STEPS) evaluations with client and caregiver in the home. Compose a plan of care for each client. The plan of care consists of a brief medical and psychosocial history, current medications, current functions and other pertinent issues. It also contains recommendations and referrals, options available to the client and the outcome. The human services specialist must make appropriate referrals to various programs. Clear concise documentation is vital. The human services specialist must maintain client files in a database.
Experience: Two or more years of professional experience in human services program areas. Long-term care and community experience preferred.
Note: The incumbent must be able to work independently, be knowledgeable of community resources, have the ability to travel through the county and be computer literate. Candidates with experience in community health/long-term care are encouraged to apply.
License: Must possess a current Maryland license as a Registered Nurse from the Maryland Board of Nursing or Licensed Social Worker (LCSW-C).
