Job Description
Nature of Work: To perform in-home assessments with adults who are disabled, older/frail. Perform interRAI evaluations with client and caregiver in the home. Compose a plan of care for each client. The plan of care consists of a brief medical and psychosocial history, current medications, current functions and other pertinent issues. It also contains recommendations and referrals, as well as other options available to the clients. Make appropriate referrals to various programs. Clear concise documentation is vital. Must enter the information in a database and maintain a file.
Minimum Qualifications
Education: A bachelor’s degree in nursing from an accredited college or university.
Experience: Two or more years of professional experience in human services or community health experience. Long-term care and community experience preferred.
Notes:
- Must be able to work independently, knowledgeable of community resources and computer-literate
- Must have the ability to travel throughout the county
- Candidates with experience in community health and long-term care are encouraged to apply
Desired or Preferred Qualifications:
- Experience working with LTSS and interRAI.
- Experience with home visiting
License: Must possess a current Maryland license as a Registered Nurse from the Maryland Board of Nursing.
