Department of Health Announces Notice of Funding Availability for Community Health Ambassador Program Community Grants

The Anne Arundel County Department of Health (DOH) is seeking community non-profit organizations interested in receiving a community grant to provide meaningful community engagement, outreach, and education aligned with the Community Health Ambassador (CHA) Program and the following six focus areas: 

  1. Screening and Referrals
  2. Access to Quality Care
  3. Behavioral Health
  4. Chronic Disease Prevention
  5. Community Safety
  6. Extreme Weather Preparedness.

The three required focus areas of the FY27 CHA Program are:

  1. Screening and Referrals
  2. Access to Quality Care
  3. Behavioral Health

Applicants should select at least one additional focus area appropriate for the population being served from the following: Chronic Disease Prevention, Community Safety, and Extreme Weather Preparedness.

CHA Program Community Grant funds may be used only to cover costs that align with the Program Focus Areas and are not funded through other opportunities. Examples of eligible activities include, but are not limited to, community meetings, training, or events; outreach activities; and staffing and staff training. Refer to the NOFA for ineligible expenditures. Grant funds will be available for a period of 1 year, with the possibility of continuation based on appropriations and performance.

Priority will be given to applications that include diversity, equity and inclusion as a component of the proposed project, to organizations that can implement the program within 90 days of award and to organizations that have staff members who received Community Health Worker Certification through a Maryland Department of Health training program.

Interested Anne Arundel County community non-profit organizations shall apply to the DOH's Office of Health Equity and Racial Justice as described in the Notice of Funding Availability. Completed applications and supporting documentation should be submitted to healthequity@aacounty.org.

  • Informational Session (optional): June 4, 2026, at 11:00 a.m. (virtual) - Register via Zoom
  • Application Due Date: June 26, 2026
  • Anticipated Award Notification: Jul 17, 2026
