Monday, February 27, 12:30 p.m. to 3 p.m. (for males and females) at the North County Health Services Building, 791 Aquahart Road, Suite 200, Glen Burnie. Sports physicals are free and by appointment only. Call 410-222-6625 to schedule an appointment. Students must bring an Anne Arundel County Public Schools "Pre-Participation Physical Evaluation" form signed by a parent or legal guardian. Click here to download the form.

Driving Directions:
From Annapolis Area: Take I-97 North to Exit 13 (Quarterfield Road). At the end of exit, make a right onto Quarterfield Road. Proceed on Quarterfield Road to the 3rd light. Make a right onto Thelma Avenue. Proceed to the next light, there is a four way intersection, proceed through the intersection (crossing over Crain Highway), the road now becomes Aquahart Road. Proceed to the next light on Aquahart Road. Make the first right after the light. The center is located on your right; it is a large tan brick building.
From Baltimore: Take I-695 (Baltimore Beltway) to Route I-97 South. Take Exit 13 (Quarterfield Road). At the end of exit, make a right onto Quarterfield Road. Proceed on Quarterfield Road to the 3rd light. Make a right onto Thelma Avenue.
