What is HIV/AIDS Case Management?HIV/AIDS Case Management is a service available for uninsured or underinsured County residents living with HIV/AIDS. Case managers are registered nurses, social workers or other trained people who provide a link between a HIV-positive person and the health care community. Case managers provide an assessment of the client's needs, and they provide referrals for services to help the HIV-positive person begin and continue with medical care and treatment. How does Case Management work?
Is there a cost? There are no direct costs to clients for HIV/AIDS Case Management Services. However, clients must be HIV-positive and meet income eligibility for some of the services arranged by case managers through referrals. Case management is available for people who are uninsured or underinsured. Ryan White Part A and B provide funds for the HIV/AIDS Case Management Program.
